Performance and Self Evaluation Forms
Performance Evaluation Forms are completed for the following types of evaluations:
Each month, department heads receive, from Staffing and Compensation Services, a listing of employees within their department whose anniversary date of hire falls in the following month and, therefore, are due to receive an annual performance review by the end of that month.
Supervisors are responsible for ensuring that employees completing initial employment, transfer, and poor work performance/habits probationary periods are evaluated as outlined in the GW Supervisors Guide.
Performance Evaluation Forms

Performance Evaluation Form for Employees in Non-Exempt Classifications: this form is authorized for the evaluation of employees in non-exempt classifications (identified by class codes 4500 or above)

Performance Evaluation Form for Employees in Exempt Classifications: this form is authorized for the evaluation of employees in exempt classifications (identified by class codes below 4500) except those with managerial/supervisory responsibilities at the salary grade 15 and above.

Performance Evaluation Forms for Employees in Managerial/Supervisory Classifications: this form is authorized for the evaluation of employees in managerial/supervisory classifications at the salary grade 15 and above.
Self-Evaluation Forms
As a complement to the aforementioned forms, it is strongly recommended that supervisors ask staff members to concurrently complete the Self-Evaluation Form and bring the completed form at the designated time for review and discussion along with the performance evaluation form completed by the supervisor. While use of the self-evaluation form is optional, its use is suggested as a tool to stimulate constructive dialogue regarding work performance. It is intended to ensure involvement of employees in their own evaluation process, to help establish directions for future activities, and to assist in continuous improvement of the quality of services.
