Employee Labels, Electronic File, or Report Request Form
The Department of Human Resource Services provides
labels, electronic files, and/or reports of paid employees
for the purpose of communicating with these employees.
To request labels, an electronic file, or a report
which includes Employee Name, Department Title, and Department Address,
complete the form and then press the "Submit" button below.
Please allow two business days for each request.
CONDITIONS

GW employee labels, files, and reports may be
used for GW campus mailing only.

Information provided may be used only for the
purpose indicated on this request form and may not be duplicated
or transferred to other groups, departments or individuals,
whether internal or external to the University.

A copy of correspondence to be mailed must be
attached to this request or specific information regarding such
listed under special instruction

Please fill out as many fields as possible to
assist us in providing targeted employee groups. A separate
form must be submitted for each request.

The Department of Human Resources Services reserves
the right to deny any requests which conflict with University
