The 2007 nomination form is available in .PDF here. The deadline was February 1, 2008. Nomination forms for the 2008 Awards will be available in November 2008.

You will need the latest version of the Acrobat reader software to view and print the .PDF form. The Acrobat reader software is available from Adobe, free-of-charge.

To request a nomination form and information packet through U.S. mail, please send your name and mailing address to Elizabeth Argeris, Flemming Awards Coordinator.

Note: This year's nomination form only requests one copy of the nomination form (not 20 as in past years).
