HEALTH INSURANCE

As an eligible employee, medical coverage is offered to you and your dependents through three plan options: a Preferred Provider Organization (PPO), a Point of Service (POS) and a Health Maintenance Organization (HMO). You choose the coverage and benefit level that is right for you. GW partners with two major health plan providers to provide these offerings.

With the PPO option, you may use any doctor or facility you wish, there is no gatekeeper and you do not need a referral to see a specialist. However, the plan will pay more on your behalf if you use in-network doctors and facilities.

The POS option allows participants to access care either in-network through a primary care physician (PCP) with a referral for specialists or by self referral to out-of-network providers (allowing you to go to any doctor or facility you choose.)

The HMO plan provides medical coverage with low out-of-pocket costs. Under the HMO plan, you must select a Primary Care Physician (PCP). All your care must be provided by your PCP or as a result of his/her written referral. Services received from out-of-network providers or facilities, or those received without a referral, are not covered except in the event of a life-threatening emergency.

Prescription coverage is included with all three of the health plans. Once you enroll in a health plan, you are automatically enrolled in the prescription plan.

Each of the medical plans offers a discount vision plan that provides discounts on exams, lenses, frames and contact lenses. Also included are a variety of wellness programs, among them nutrition, smoking cessation and healthy pregnancy.

DENTAL INSURANCE

Eligible employees are also able to participate in the Group Dental Plans offered by GW. The University has two dental PPO options and a Dental Maintenance Organization (DMO) option.

Services are divided into three groups: preventive, basic and major. Your reimbursement will depend on the option in which you participate and whether you received services in or out of the designated network.

The high dental PPO option provides participants with a high level of coverage at a competitive cost. You can select the dentist of your choice, but seeing a dentist participating in the plan reduces your costs.

The low dental PPO option provides participants a lower level of coverage and, as a result, will cost you less. The low option does not provide coverage for major services. As with the high dental option, you can see any dentist that you wish but you will receive a higher level of reimbursement by using a plan-participating dentist.

The DMO provides dental coverage for your preventative, basic and major services for a fixed-co-pay amount.

SHORT TERM DISABILITY INSURANCE
GW offers a voluntary short-term disability (STD) plan. You can purchase this coverage equal to 60 percent of your weekly base pay to a plan maximum.

LONG TERM DISABILITY INSURANCE (Only available to full-time employees)

After one year of employment service, GW makes available long-term disability (LTD) coverage to all full-time employees at no cost. The plan pays 60 percent of your monthly base pay up to a plan maximum. Employees also can elect the LTD Buy Up plan. Participation in this Buy Up plan is completely voluntary. The Buy Up plan pays an additional 6 2/3 percent of your monthly base pay (on top of your LTD 60 percent) up to a plan maximum.

FLEXIBLE SPENDING ACCOUNTS
The University offers a flexible spending account program, which allows eligible employees to set aside tax-free dollars in special accounts to pay out-of-pocket health and/or dependent care expenses.

You can contribute to one or both accounts. Participation is voluntary and you need not be enrolled in any of the GW health plans to participate.

LIFE INSURANCE
Group Term Life and Accidental Death & Dismemberment (AD&D) coverage is provided at no cost to employees in an amount equal to the base annualized salary, up to a plan maximum.

Supplemental Life Insurance Plans

Supplemental group term insurance is also available for you, your spouse/same sex domestic partner, and dependent children.

VOLUNTARY LEGAL SERVICES

Employees have the opportunity to participate in the group legal plan. A variety of legal services are available to members through participating attorneys for a monthly fee. This plan offers coverage for specific services and partial coverage for others.
