Internship/ Field Experience Application Instructions for ALL students

In order to be considered for an internship placement, you must submit a complete student teaching placement packet. Incomplete packets will be returned to the student and may cause delays in obtaining an internship placement. A complete packet includes:
Ø George Washington University Internship Application Form
Each program has a different form. Click here for a list of forms and programs. You must
get application form signed by your advisor. Placement requests without an
advisor's signature will not be processed.
Ø School System
Application Form – (Fairfax, DC, Arlington, Montgomery,
etc..)
Note:
Alexandria City, Falls Church City and Stafford County, do not have their own
forms.
Loudoun
County’s form can be accessed through their website
and then follow these instructions:
Ø Complete the Online Application and Submit
Arlington County
requires two letters of recommendation with their application – no exceptions!
Ø Resume
School Systems ask for a current resume for
their files. Please make sure GWU is
listed as your current school of attendance.
Ø Biosketch
A well written, short biography, outlining your
educational background and experience.
Ø TB Verification
This may be obtained from the Student Health
Center or from your doctor. Tests typically cost $15. Students cannot start an internship
unless their TB test is on file with OLE. If you currently hold proof of
negative TB, the results cannot be older than 9 months.
Ø Transcripts
Most recent unofficial copy showing your GWU
coursework. Undergraduate transcripts and other graduate transcripts can also
be helpful in providing additional information about your content area, but
they are not required.
Ø
Background Checks
DCPS requires
a background check prior to placement.
Students (fieldwork and internship) need to take a copy of the Volunteer
Application and a copy of the TB test results
to 825 North Capitol Street, 6th Floor to be
fingerprinted at least 6 weeks prior to the start of placement.
Arlington
County requires a background check upon placement confirmation. Once you have received your school and
teacher contact information, please immediately call
or email Ms. Erin Wales-Smith to arrange for your
fingerprinting and ID badge. 703-228-2419 | ewales@arlington.k12.va.us
Alexandria
City requires a background check upon placement confirmation. Once you have received your school and
teacher contact information, please immediately
schedule the processing appointment with Debbie Gardner by calling her
directly at 703-824-6697. The “processing”
procedure takes approximately 15 minutes and
includes
fingerprinting and completing the paperwork for the criminal record check and a
search in the child protective services central registry. Students should
identify
yourselves
as an intern and be prepared to pay a $42 processing fee.
Prince George’s County requires
a background check upon placement confirmation.
OLE will provide a letter for you
to take to the fingerprinting office to verify that you are eligible for
fingerprinting. Fingerprinting occurs at
the Prince George’s County Public Schools Fingerprinting Office (301-952-6775)
in the Sasscer Building. The cost for fingerprinting is $61.25. Interns
should take a copy of the receipt received from the Fingerprinting Office to OLE. Instruct
interns to place Ms. Marable's email address on the fingerprinting form to have
the results also sent to her office. (constance.marable@pgcps.org)
Howard County requires
a background check prior to placement.
v Go to the the
nearest State Police Barracks (http://www.mdsp.org/fob/barrack_choices.asp)
at on the designated fingerprinting days.
Each barracks will have their own schedule. Get
there early. They only do fingerprinting on this day/time every week.
v Present valid,
government-issued photo identification
v Bring a personal
check or money order made payable to 'Maryland State Police' in the amount of $10.00
- this is per fingerprinting card and you’ll need two.
v They will take your
fingerprints and give the cards (blue and pink) back to you to mail
to the Department of Public Safety and Correctional Services Criminal Justice
Information System.
v The pink CJIS form will
ask you for this information:
o
Authorization number: 0800005570
o
Below the authorization number, list OLE (Suzy Wise) as the
office to receive the results. Our
address is 2134 G Street, Suite 401 – Washington, DC 20052.
o
Please check ‘state and fbi
volunteer’ and ‘child care’.
v Include a check
made payable to CJIS – Central Repository in the amount of $33.25 and
mail the forms to CJIS – PO Box 32708 – Pikesville, MD 21282-2708.
Please understand
that although we can request placement in the school of your choice, we cannot
guarantee it. The final decision lies with the School System Office.
